Requirements

Complete the online registration process with accurate and up-to-date information.
Agree to the Terms and Conditions.
Approval by PetfolkFlex administrators and/or Pet Care Center leadership.
Meet all requirements specified for each shift, including holding a state veterinary license in good standing.
Possess an active Drug Enforcement Administration license (DEA license).
Have active Professional Liability Insurance (PLIT) coverage.
In South Carolina, hold an active Controlled Substance license.
Provide an up-to-date W9 for billing purposes.
